Embrace Healthcare is proud to offer diverse options for in-home and out-of-home respite. Our in-home service puts you first, providing skilled care workers that come to your home and support you. Our out-of-home respite, offers a comforting atmosphere and much-needed rest. No matter where you are or what your situation is, Embrace Healthcare Solutions will be your helping hand to assist you in taking control of your life again.
It is important that you like your carers and establish a friendly rapport with them. We will usually send you your preferred carers, but sometimes your carers may not be available due to sickness or their leave arrangements. In this case, we will only make a substitution if you agree to it.
That depends upon your care plan and your requirements. If your preferred carer is available at the time you require them, then of course this will be arranged.
Given sufficient notice, it can be scheduled for your preferred carer to be available. If this is not possible, another qualified carer will be arranged to assist you, as long as you approve them.
Bathing, dressing, toileting, and grooming for up to two hours per day, including bowel management, skincare, bladder management, and menstrual management; first aid and medication administration; exercise, positioning, and moving for up to one hour per day; mobility assistance for up to two hours per day.
Daytime Self-Care Assistance – Standard – Weekdays. By assisting and supervising the participant’s daily tasks, the participant will learn to live independently.
In what sense does a daily personal activity with high intensity mean? Participants, workers, and others who participate in high-intensity daily activities are at higher risk. NDIS providers must take special care to hire skilled workers with the appropriate knowledge and skills to provide high quality and safe supports.
In the capacity building category, Improved Daily Living is included. A subcategory of this category is allocating funding for training and support to help individuals maintain or increase their skills to remain independent and participate in their communities.
Help with your daily bathing routine to feel confident and refreshed. As part of these supports, people can receive assistance dressing, showering, and toileting. Depending on the level of support, Self-Care can be classified as Assistance with daily activities at home, work, or community.
People in need of assistance with wound care or pressure care are provided with support workers. Nursing professionals are typically responsible for wound care.
Whether you live alone, are active in employment, educational, or social activities, or are otherwise away from home, personal activity support assists with daily activities. Such activities include supervision or assistance with daily living activities.
The Improved Daily Living Skills budget category includes the Core Supports and Capacity Building categories. By developing these skills, participants can live independently and require less assistance.
The NDIS enhances participants’ ability to travel independently. A ndis transport support may include equipment and aids related to personal transportation or training in public transportation.
A support worker can also assist you in the community if you need a ride. You will have to pay for additional travel costs, such as road tolls, parking costs, or public transportation tickets.
As part of the standard claim, providers must also charge travel time. Travel time claims should be identified as the claim type. Amounts should reflect the actual amount of travel time. For example, it is possible to bill only 10 minutes for travel time.

Support Coordination is an NDIS funded service that’s designed to help NDIS participants (and, if required, their family or representatives) to coordinate the support services in their NDIS Plan.
If you’re living with disability, your plan from the National Disability Insurance Scheme (NDIS) gives you a set allocation of funding for a range of different support services.
The support funding in your plan is very specific and relates to helping you to achieve your personal goals. This might include a range of services such as in-home support, social and community access or therapy sessions. Your NDIS plan will state if you are eligible for Support Coordination.
Support Coordination is designed to work with NDIS Participants to build and maintain a support network. This involves:
supporting you to understand your NDIS plan;
assisting you to have choice and control over your NDIS services and other supports;
assist to get you through any challenges that might come up with your support services; and
capacity building.
Over time, the Support Coordinator will help their Participant to build the skills (and confidence) to exercise the choice and control over the management of their Plan. It’s about aligning the Participant’s own goals to ensure the Support Coordinator empowers them with all that is possible.
Supports include informal, mainstream, community and funded support. This could be occupational therapy, social support, in-home support, job support, educational support, community engagement (to name a few).
Yes. There are three different types of Support Coordination.

The type of funding you receive will depend on your individual needs and will be outlined in your plan.
A Support Coordinator will help to connect you with all the individual services outlined in your plan.

No. A Support Coordinator’s role is to ‘translate’ your NDIS plan and to connect you with the people and services to achieve your goals.
A Plan Manager will organise payments, invoices and manages your budget.
Yes. Support Coordinators can help navigate this process. Before the meeting, Embrace Healthcare Support Coordinator can produce a Plan Review Report to outline the services arranged. This will include assessment reports to demonstrate progress during the Plan period as well as recommendations for future services. During the meeting, Embrace Healthcare Support Coordinator will support Participants and their representatives. Afterwards, they act as a point of contact for the NDIS.
An independent Support Coordinator is able to find the exact service to meet your individual needs and goals. At Embrace Healthcare Solutions, we are not bound by any particular services we must use. Our purpose is to match you to the best service to help you thrive. If something’s not working, we change it. No. The NDIS fully pays for your Plan Manager in addition to your regular disability funding. The extra money you receive for plan management will not lower your budget for care and therapy.
If you are plan managed, an extra funding category will be added to your plan. This does not impact other areas of your NDIS funding and is free of charge for you.

No. Under NDIS rules, every NDIS participant has the right to nominate a Plan Manager. You don’t need to explain why you need a Plan Manager and the NDIS cannot refuse your request for one.
A plan manager will help you along your NDIS journey by processing invoices, ensuring that providers are charging in accordance with NDIS guidelines and that you are spending within your budget.
A support coordinator will connect you to local providers, help you with your plan review process and submit requests to the NDIS for any changes or approvals you may need.
No. You have a full range of options with a plan manager and can choose to use registered and unregistered providers.
You can change plan managers at any point during your plan. You will need to notify your current plan manager and confirm your release date. Make sure you have another plan manager lined up to avoid delays in invoice payments.
You will be matched with a personalised plan manager. Your plan manager will contact you to introduce themselves and set you up on the Provider Choice dashboard. The dashboard will give you access to all invoices that we process for you, funding updates as well as some great free tools to help you along the way.

For you to receive nursing care services t funded under the NDIS your NDIS Plan must first include Capacity Building in it.
If you receive nursing care services that are not part of your NDIS Plan, then you will need to fund those services yourself or receive them through the State health care system.
The types of services that can be included in your plan are those that would contribute to your ability to live independently.
All NDIS funded nursing services must:
· be related to your disability
· contribute to your long-term wellbeing
· be delivered by a nurse and
· be limited in time.
The NDIS won’t fund nursing services that:
· can be funded under the state health care system
· are not directly related to your disability.
· are not required to be delivered by a qualified nurse
· are not considered ‘capacity-building’ that is, they don’t contribute to your independence or build up your skills towards independence
Diabetes is considered to be a disease rather than a disability and on its own does not qualify for NDIS funding. However, if you have lost limbs or suffer from blindness due to diabetes then you may be eligible to receive support through the NDIS.
If you have epilepsy you may be eligible for funding through the National Disability Insurance Scheme (NDIS). This comes as a surprise to many people with epilepsy, as they often don’t consider their epilepsy to be a disability. However, a person with epilepsy may be eligible for support depending on the impact the epilepsy has had in the functional areas of life such as mobility, communication, social-interaction, learning, employability, ability to self-care and self-manage.

Occupational Therapists support people to live a more independent life. They can support your ability to perform everyday tasks in your home, improve access to your community, help you to explore housing or driving options or find out about technology that can assist you with mobility and access.
Occupational Therapy is a degree-based health profession, regulated by the Australian Health Practitioner Regulation Agency (AHPRA). Occupational Therapists use a whole person perspective to work with individuals, groups and communities to achieve optimal health and wellbeing through participation in the occupations of life.
The term ‘occupation’ is used to describe all the everyday things we do in our life roles, but also the things we do to be who we are, the things we do to create a meaningful life and to engage with wider society and culture.
Whatever you need, our Occupational Therapists will assess your situation and provide personalised services to help you do the things you want and need to do to, in the most effective way possible.
A Functional Capacity Assessment (sometimes referred to as a Functional Capacity Evaluation) is an independent assessment of your function, capacity and needs. Its purpose is to make sure you have the right level of support and funding to achieve your goals and needs.
Ability Action Australia clinicians are trained to conduct objective assessments, using standardised measures where required. We take into account your function, supports and daily living, vocational and community abilities, and goals and aspirations.
Functional Capacity Assessments include a face-to-face or telehealth component, communication with involved parties (to ensure consent) and a report that is provided to both you the participant, and the NDIA. The process assists with decision making for the type and level of funding and supports under an NDIS plan.
Psychosocial Recovery Coaching is an innovative approach to recovery that focuses on improving mental illness, physical health, self-confidence and self-esteem. By engaging with a certified Psychosocial Recovery Coach, individuals can obtain the guidance and support needed to design and implement their own customized plan for recovery. Through this process, detailed evaluation and assessment of individual needs are conducted to identify and manage any behaviours of concern while striving towards optimal functioning. This may include behavioural interventions, anger management or strategies for managing self-injurious behaviour. In addition, Psychosocial Recovery Coaches also provide assistance with the coordination of NDIS plans and other supports as well as training carers in effective behaviour management strategies. By leveraging the power of collaboration and creative problem-solving between coaches, carers and clients, we aim to create meaningful long-term change on a personal level that leads towards real life goals.

The National Disability Insurance Scheme (NDIS) is designed to support people with disabilities. Its aim is to support them so they can increase their independence and fully participate in community and working life. People who meet the NDIS access criteria are known as NDIS participants.
Not everyone who has a mental health condition will have a psychosocial disability, which is the term used to describe a disability that might arise from a mental health issue. It can be severe, long term and impact their ability to recover and live life independently.. People who do have psychosocial disability may qualify for the NDIS.
It can be hard for someone with ADHD to meet the NDIS eligibility criteria for a severe and permanent disability.Therefore, the NDIS does not usually cover ADHD.
The NDIS does provide funding for persons with ASD. The autism spectrum disorder is the largest primary disability category for the NDIS. There are certain conditions that need to be met however, so not all individuals with autism spectrum disorder will be approved for NDIS funding.
A behaviour support plan is a document prepared in consultation with the person with disability, their family, carers, and other support people. It addresses the needs of a person who has been identified as having complex behaviours of concern.

Your eligibility to access the NDIS is determined by the National Disability Insurance Agency (NDIA). The NDIA use the information provided by you or your loved one in your application to make this assessment.
According to the Access Checklist provided by the NDIA, to be eligible for the National Insurance Disability Scheme (NDIS), you must be:
An Australian resident, Australian citizen or hold a Protected Special Category Visa
Under 65 years old
Live in an area where the NDIS is available
Currently require support from people or equipment, or require supports now that will reduce your support needs in the future

To access NDIS funding, you need to be less than 65 years when you first access the scheme. If you are over 65, you may be able to access other aged care supports, such as My Aged Care. For more information on My Aged Care, please visit their webpage.
Yes. You will receive funding annually to purchase services and supports. You then have the option to self-manage and claim directly from the NDIS, or you can have some help to manage the plan and funding. If you do not want to engage someone to help you manage the plan, you are able to self-manage.
Support coordinators work closely with you to identify needs, personalise supports and make sure you are matched with the right services.

NDIS payments to participants are GST-free if all of the following requirements are met:
The participant has an NDIS plan in effect
The supply is of reasonable and necessary support that are specified in the statement of supports in the participant's NDIS plan
There is a written agreement with the NDIS participant
The supply is covered by one of the tables in the New Tax System (Goods and Services Tax) (GST free Supply—National Disability Insurance Scheme Supports) Determination 2021
If you are a participant, or a nominee of the participant, under the National Disability Insurance Scheme (NDIS):
Payments you receive are tax free.
You can't claim deductions for expenses or assets paid for by the scheme
You may also have tax and super obligations in certain circumstances.
The NDIS will not fund:
Medication
General medical and dental services and treatment
Specialist services
Hospital care
Surgery and rehabilitation.
The NDIS will not cover things that are unrelated to your disability.
The NDIS can help pay support funding for individuals based on their needs, including daily personal activities, transport and mobility (such as wheelchairs), access to work and education, household tasks, home and vehicle modifications and therapeutic support. NDIS can also pay for home and vehicle modifications for eligible participants.
In most cases, the NDIS does not provide funding for the participant’s housing costs such as rent.

Every NDIS participant has an individual plan that lists their goals and the funding they have received. Every participant uses their funding to purchase supports and services that will help them achieve their goals.
All terms written in a signed service agreement are legally binding.
An NDIS registered provider is an individual or organisation delivering a support or a product to a participant of the NDIS. Organisations or individuals can apply to be a registered NDIS provider with the NDIS Quality and Safeguards Commission (NDIS Commission).
The NDIS Worker Screening Check is an assessment of whether a person who works, or seeks to work, with people with disability poses a risk to them. The assessment determines whether a person is cleared or excluded from working in certain roles with people with disability.
NDIS (National Disability Insurance Scheme) - is a scheme run by the Australian government to provide funding for Australians living with disabilities.
NDIA (National Disability Insurance Agency) - manages the NDIS. Funded by the Department of Social Services, and appointed by the Australian government, the NDIA is responsible for administering all NDIS funding.
There are three options to manage your NDIS funding - self-managed, plan-managed and NDIA-managed.
SELF-Managed Plans:
This is when you manage your own NDIS funding provided by the NDIA. You have the flexibility and choice to decide which supports you will buy to help you meet your goals.
PLAN-Managed:
In this option, the NDIA provides funding in your plan to pay for a Plan Manager who then pays your providers on your behalf, they keep track of the funds and your financial reporting.
NDIA-Managed Plans:
This is also referred to as being Agency-managed, it is when the NDIA manages your plan. While you are able to choose from a range of registered NDIS provid
A person ceases to be a participant in the NDIS when:
the person dies
the person enters a residential care service on a permanent basis and this first occurs after the person turns 65 years of age.
From 25 October 2021, any person who provides disability services must have at least one dose of a COVID-19 vaccine. These workers must have had two doses of a COVID-19 vaccine by 29 November 2021.
This includes people who:
work in residential disability care
provide in-home disability services
provide disability day programs
provide community-based disability support services
any person providing services funded or provided by:
the National Disability Insurance Scheme
the Assisted School Travel Program.
Workers will need to provide proof of vaccination to their employer if requested.

Am I Eligible?
To access the scheme, potential participants must first meet certain eligibility criteria. To be eligible for the NDIS scheme you must: be an Australian citizen or permanent resident; have a permanent disability; need help with everyday tasks; live in Australia; and be aged under 65 years old. Once you’ve been assessed as eligible for the scheme you will then need to develop an individualized plan outlining your goals and objectives for life under the scheme. This plan will need to be reviewed regularly so that it remains up-to-date with your changing needs and circumstances. How Do I Access NDIS?
If you meet the eligibility criteria you can ask to become a participant by completing an Access Request Form. If you are already receiving disability support services, the National Disability Insurance Agency (NDIA) will contact you when it becomes available in your area.

How is my NDIS plan developed?
Once the NDIS confirms you are an eligible participant, a planner will arrange to meet with you. Attending a planning meeting, either in person, or over the phone, is the best way for the NDIS to gather all your information to develop the best plan for you. During your planning meeting, make sure you discuss the goals/activities/tasks you want to achieve with your ECEI Coordinator, LAC or NDIA planner. They will undertake a full review of the supports and services you currently receive. Using the information discussed, your planner will develop a support plan that best meets your needs and goals. Embrace Healthcare would also welcome any opportunity to talk throughout the planning process to discuss your goals and potential support requirements.
How do I select my NDIS service provider?
When you have an approved NDIS plan, you need to understand the different budgets and who can help you start using your plan, and how your plan is managed. It is now up to you to choose what supports and services you feel you need to achieve your goals, who provides them and when and where they are delivered. When you’re working on your support plan, you can nominate Embrace Healthcare Solutions at any time as your service provider. Once your support plan and budget are approved, you then select your service provider. If Embrace Healthcare Solutions is selected, in accordance with the plan, we then develop and implement your “person centred” support plan working closely with you and your team. The courses are delivered online or virtually with some components requiring a practical or short theory assessment. Our teams are encouraged to attend one session each quarter and the session dates and times are shared 3 months in advance, so our team has plenty of time to schedule their training.
The great news is that the Training and development at Embrace Healthcare solutions is Subsidised or paid on-the-job training. This means all of our staff are paid when they attend a session.

With over 10 years of experience in the mental health nursing field and a strong passion for making a difference, Sam has dedicated his career to improving the quality of life for individuals with disabilities. As a loving spouse and proud parent of two boys, Sam enjoys spending time with his family camping, reading the bible, playing squash, and walking. Sam enjoys long-distance driving and taking in the beauty of the outback. Sam has lived and worked in rural and remote Western Australia for two years, gaining a unique understanding of the unique challenges faced by individuals in these communities. His attention to detail, combined with his clinical experience and delivering training, has made him a valuable asset in delivering therapeutic sessions and group training for people with cognitive impairment. Before entering the healthcare field, Sam honed his skills in financial management, stakeholder relations, and logistics in his business ventures in Zimbabwe. This business acumen has proven to be an asset in Sam's current role as the owner of Embrace Healthcare Solutions, where he can combine his clinical knowledge, educational expertise, and passion for inclusivity and empowerment to make a lasting impact in the lives of those he serves.

Amelia is a highly motivated and compassionate Occupational Therapist with a wealth of experience in the assessment, treatment, and rehabilitation of individuals with disabilities. With a deep understanding of Australian Standards AS1428, Amelia can provide patient-centric care and support, using her skills and knowledge to empower and improve the quality of life for her clients. Throughout her career, Amelia has gained hands-on experience in various clinical settings and has always been committed to continuous learning and professional development. This has allowed her to stay current with the latest research and best practices in the field of Occupational Therapy, and to be at the forefront of assistive technology. In addition to her work as an Occupational Therapist, Amelia is also an avid horse rider and fisherman and enjoys the thrill of dancing salsa in her free time. Recently, she has taken on an exciting new challenge - building a mobile software application. Having lived in the Pilbara for two years, Amelia has a unique perspective on the unique needs and challenges faced by individuals with disabilities in regional areas and is dedicated to making a positive impact in these communities. With her passion, commitment, and professional experience, Amelia is poised to make a meaningful difference in the lives of those she serves.
